Planning for End of Life

Putting Plans in Place

Talking about death and dying can be difficult for many people.  This isn't an easy subject to discuss, but it is important for all of us.  By talking about it we can begin to explore what matters to you, and how to put plans in place.

We have invited Ann to raise awareness of the need to plan and to provide information that can be very useful.

This will help you make choices and share your wishes with your GP and others close to you.

In addition to Ann’s training through Gentle Dusk, she has been a ChildLine counsellor for more than 17 years, she is a good listener and would like to support you.  It is important to us that you receive the care and support you wish to receive.  Whatever you discuss is confidential between you, your GP and Ann.

Please give your name to reception and Ann will contact you.

She very much looks forward to meeting you.
