New Patient Registration
Patients now register with the practice and not a particular doctor. A patient can express a preference of doctor, but this is not always possible. We accept patients from within our practice catchment area.
If you wish to register with us, please visit the surgery. To avoid delays or disappointments, please ensure you have all the appropriate documents with you when you come in. We require an ID (passport), NHS card, utility bill or a bank statement to fully register you as a patient (see full list of documents required below).
Medical treatment is available from the date of registration. Please contact reception for further information.
Patients can register on Monday to Friday between 9.30am to 12pm and between 3.30pm to 5.30pm.
Nationality is not relevant in giving people entitlement to register as NHS patients for primary care services. Anyone who is in the UK may receive NHS primary medical services at a GP practice. There is no set length of time that a patient must reside in the UK to become eligible to receive NHS primary care services.
A patient does not need to be “ordinarily resident” in the UK to be eligible for NHS primary care – this only applies to secondary care. The length of time that a patient is intending to reside in an area in the UK dictates whether a patient is registered as a temporary or permanent patient. Patients should be offered the option of registering as a temporary resident if they are resident in the practice area for more than 24 hours but less than 3 months.
This includes asylum seekers and refugees, overseas visitors, students, people on work visas and those who are homeless. Practices can only refuse an application to join their list if the practice has “reasonable grounds for doing so that do not relate to the applicant’s race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition”.
Reasonable grounds include:
- Patient previously removed from the list following the breakdown of the GP/patient relationship
Overseas visitors, whether lawfully in the UK or not, are also eligible to register with a GP practice even if those visitors are not eligible for secondary care services. Patients should be offered the option of registering as a temporary resident if they are resident in the practice area for more than 24 hours but less than 3 months (or immediately necessary if clinically appropriate).
IMMEDIATELY NECESSARY TREATMENT
General Practices are also under a duty to provide emergency or immediately necessary treatment where clinically necessary irrespective of nationality or immigration status. They are also required to provide 14 days of further cover following provision of immediate and necessary treatment.
When registering children (under the age of 16), please note that we require the sight of child's Birth Certificate and the Red Book or equivalent. If a child is adopted, we will need to see Adoption Documents and if a visiting child is staying with the relatives, we will need to see documents from the parents’ giving authority to the relative/s.
DOCUMENTS REQUIRED FOR PERMANENT REGISTRATION
If you are wishing to Register with the Practice, you will be required to provide the following Documents for Registration purposes.
- Current Passport
- One Proof of Address
Proof of address must be recent (last 2 months) and may constitute a Bank Statement; Utility Bill or Tenancy Agreement.
If you are a member of Domestic staff and have no recent proof of address, you will be required to furnish your Passport and a letter from your Employer, on their official letterhead that you are currently employed by them. Please be aware only Employers who are Registered with this Surgery may provide such a letter in support of your application. Where an Employer is not registered with this practice, you will need to Register with their NHS G.P.
- NHS Number and Name and Address of your previous G.P.
If you have been Registered with a G.P. prior, you will be required to furnish your NHS number. In addition the name and address of your old G.P. is also helpful in locating your old medical records.
With respect to children, you will be required to bring in the Red Book for them so that the Nurse can update all the immunisations on our clinical system.
The practice would like to congratulate you on your recent bundle of joy and would like to guide you in that it will require the Full Birth Certificate and Red Book in order to get baby Registered.
- Registration of Asylum Applicants
A Home Office letter will be required confirming your Asylum status.
- Registration based on Spouse/Employment Visa
When Registering on the basis of a Spouse/Employment Visa, the Dependant Visa Holder must also be Registered with this Practice.
